Correct. Damage remains between form-changes (and so do Wounds), you use the character's Health Rating (including Wound Threshold).

It was just noticed in Discord chat that the 4e map has the same "days walking riding, airship traevl distance" scales copy&pasted from the 3e map. The 3e map shows less territory overall, and this means it's key is the wrong scale for the 4e map. The correct scale of the key is as shown in my Atlas...

I really love the ideas here. We played around with spellcasting-as-a-skill in 1e, but left it behind when the campaign went to 2e. I'm really liking the idea of attaining a 0 thread spell to a matrix object and handing it to another spellcaster. There's really nothing in the metaphysics that spoils...
